Addendum

Formal Definition

A supplemental addition to an existing medical note that provides clarification, correction, or new information, appended after the original note has been signed, without altering the original content.

How It's Used on the Ward

"Add an addendum" or "I'll addend the note" — used when information was omitted, a result returned after signing, or a correction is needed.

Example

""Signed the discharge summary and then the final culture resulted: positive for MRSA. Added an addendum: 'Addendum 16:42 — final blood culture positive for MRSA. PCP notified, ID follow-up scheduled at 1 week, repeat blood cultures ordered.'""

Clinical Context

An addendum supplements; it does not replace or delete the original note. The original timestamp and content remain visible in the chart. This is a legal and compliance requirement — backdating or altering original notes is a HIPAA/medical records violation and grounds for termination. When correcting a factual error (wrong medication dose documented), the addendum must clearly state what was incorrect and what the correct information is.
