Formal Definition
Altered Mental Status - any change in a patient's level of consciousness, orientation, or cognition.
How It's Used on the Ward
"Patient has AMS" - can mean anything from mild confusion to unresponsive.

""72-year-old presenting with AMS. Found down at home. GCS 10. Differential: stroke, infection, metabolic, intoxication, head bleed.""
Clinical Context
AMS is a symptom, not a diagnosis. Always figure out WHY. The mnemonic AEIOU-TIPS covers the major causes.
