Appy
Formal Definition
Appendectomy: surgical removal of the vermiform appendix, typically performed laparoscopically for acute appendicitis — one of the most common emergency surgical procedures.
How It's Used on the Ward
"Classic appy presentation" or "taking her to appy" — shorthand for the procedure and diagnosis.
Example
""Classic appy: 22-year-old, RLQ pain migrating from periumbilical, anorexia, low-grade fever, WBC 13, CT shows 10mm appendix with periappendiceal fat stranding. Laparoscopic appendectomy scheduled. Antibiotics started pre-op. Expected discharge POD1.""
Clinical Context
Appendicitis diagnosis: clinical (Alvarado/MANTRELS score), supported by CT (sensitivity ~98%). Ultrasound first in children and pregnant women to avoid radiation. Non-perforated: laparoscopic appendectomy, same-day or next-day discharge. Perforated appendicitis: longer OR, IV antibiotics, drain placement, prolonged course. Medical management (antibiotics alone) is emerging as an option for uncomplicated appendicitis — studies show ~30% recurrence at 1 year, but many patients avoid surgery. The McBurney's point, Rovsing's sign, and psoas sign are classic exam findings every student should know.
