DC (discontinue)

Formal Definition

Permanently stopping a medication or order. Not to be confused with "hold."

How It's Used on the Ward

"DC the Foley" means remove it permanently. "DC the heparin" means stop it entirely.

Example

""Patient's AKI is resolving, creatinine back to baseline. DC the nephrology consult and discontinue the IV fluids.""

Clinical Context

Distinguish DC (permanent) from Hold (temporary). Wrong one creates confusion. When discharging, DC any unnecessary inpatient meds before writing new outpatient regimen.
