Formal Definition
Temporarily discontinuing or pausing a medication order, usually because of a clinical change or procedure.
How It's Used on the Ward
"Hold the metformin" means stop the medication until further notice. Always communicate the reason.
Example
""Patient's creatinine jumped to 3.2 overnight. Hold the metformin and the ACE inhibitor and check a BMP in the morning.""
Clinical Context
Always know WHY a med is being held and document it. "Hold" is different from "DC" (discontinue) — holds can be restarted.
