Q-shift
Formal Definition
Once per nursing shift: a documentation or monitoring frequency indicating that an assessment, vital sign measurement, or nursing task should occur once per 8 or 12-hour nursing shift.
How It's Used on the Ward
"Check it q-shift" or "weights q-shift" — less frequent than q8h in intent, linked to shift handoff rather than clock time.

Clinical Context
Q-shift and q8h are often used interchangeably in practice but q-shift implies alignment with nursing workflow (nursing shift change). It's practical for assessments that are most meaningful at consistent, predictable times — daily weights in CHF, wound checks, drain output totals. Ordering at the right frequency matters: q4h skin checks cause alarm fatigue; q-shift wound assessments catch deterioration in time to intervene.
